One of the first national Local Health and Care Record (LHCR) exemplars (also known as LHCRE) the TVS Care Records Partnership is connecting and sharing information through the work of the programme to improve your health and care across the Thames Valley and Surrey (TVS) area. The TVS programme is developing a digital system to connect local records across the area to benefit patients in the NHS, health, community and social care.

The TVS Care Records is a way of sharing patient information with health and care staff. It means information recorded about your health such as illnesses, hospital admissions and treatments can be accessed by different people who are involved in your care, wherever you are seen in Buckinghamshire, Berkshire West, Frimley, Milton Keynes, Oxfordshire and Surrey.
By sharing this information safely, quickly and securely using the TVS Care Records, health and care workers can access the most current details about you to make the best choices about your care.
