As vendors and visitors are allowed to return to our locations for visits we must ensure the best safety practices are in place to protect our work force. The visitor questionnaire will be used to screen each vendor/visitor that comes on any of our properties. Each vendor/visitor must fill out this form before being allowed to enter. A copy of the completed form will be kept on file for future reference.
