The 17-year gap between research and practice is well known. Scientific ideas developed in efficacy and effectiveness trials can take a long time to be actively disseminated into practice-based settings.
This is not just a top-down problem. Community-based organizations report needing guidance to implement evidence-based change strategies. These same organizations may also have homegrown processes and adaptations that work as well or potentially better for their patients and stakeholders than published interventions.
