Example domain paragraphs

The FQHC Telehealth Consortium seeks to eliminate health and digital disparities by building a model of sustainable, patient-centered, and equitable telehealth practices throughout Massachusetts—and across the country.

Founded in March 2020 by Community Care Cooperative and the Massachusetts League of Community Health Centers, the Consortium helps health centers across Massachusetts pivot to sustainable models of telehealth, integrate them into their primary and behavioral care practices, and ensure financial viability.

Today, the Consortium includes 35 Federally Qualified Health Centers (FQHCs) and serves more than 700,000 patients from some of the Commonwealth’s most racially and ethnically diverse communities.
