In order for DBC to prepare for your first visit, you will need to contact us at the email address below. Afterward, we will send you an online Personal Data Inventory (PDI) form to fill out with the following information
You will then be matched with two counselors. One of them will contact you to schedule a Zoom meeting to review the PDI form and begin your counseling sessions.
