The Community Pharmacy Future project was created in 2011 by Boots UK, LloydsPharmacy, Rowlands Pharmacy and Well (formerly The Co-operative Pharmacy). We set out, and achieved our aim, to design pathways for pharmacy services that will give patients and carers the practical support they need for getting the best outcomes from medicines that have been prescribed for long-term conditions.
