Description: Conscience Community Network (CCN) is a network of Illinois nonprofit family social service organizations that works with at-risk youth and families to identify and address their unmet needs. We utilize a model of strengths-based, community-based care called "Wraparound" which identifies needs, strategizes on how to address those needs, and collectively works together to meet those needs.
