Dear Community Leader, Cook Children’s needs your help with the following confidential survey about children’s health in our area. Every three years, we conduct a Community Health Needs Assessment (CHNA) to identify the children’s health needs in our community and develop solutions with community partners to address the most important issues. We need information from community leaders in the following counties (Tarrant, Denton, Johnson, Hood, Parker, Wise, Grayson and Collin).
In addition to this survey, the CHNA includes a household survey of parents and caregivers of children ages 0-17, qualitative data from families, and a review of facts from government and other health organizations. The unique perspective that you provide as a community leader helps us integrate all of this important information, and develop a plan to collaboratively address the identified health priorities. This information also enables us to track our progress in addressing health needs over time.
